Some general guidelines for applicants are worth stressing here.

Read the eligibility guidelines carefully, so that you do not waste your time and money by applying if you are not likely to be eligible in the first place. If in doubt please contact the Secretary of the Accreditation Committee to enquire.

Make sure that your application contains all the necessary information, and is written as clearly as possible.

If in doubt about any aspect of ASAB Certification or your application then please do not hesitate to contact the Secretary of the ASAB Accreditation Committee.

The application form for currently-active practitioners with extensive clinical experience who wish to apply for full certification in 2008 is available here as a Word document for downloading (preferred), as is the application form for practitioners with only moderate clinical experience who wish to apply for pre-certification. Paper copies of both can also be obtained from the Secretary of the ASAB Accreditation Committee.
